New Patients

I accept new patients for craniosacral therapy and injection therapies on a case-by-case basis. To request a new patient visit, please email shorelineclinic@gmail.com.

Your first visit may last up to an hour and a half.  

New patient forms need to be completed before your first appointment.  Please complete them and email them to the office or bring them to your appointment, along with your insurance card. You can download the forms using the links below.

 

I accept Regence Blue Shield, Premera Blue Cross, Uniform Medical, and Lifewise Health Plan of Washington.  All plans offer differing coverage for naturopathic medicine, manual therapies such as craniosacral therapy, and injection therapies such as trigger point injections.  Check with your insurance to be certain you understand your coverage.  

I am not a preferred provider with and do not bill Kaiser, First Choice, Aetna, Cigna, United Healthcare, Medicare, Coordinated Care, Molina, Community Health Plan or Apple Health. 

I do not manage Labor and Industries cases.

I do not manage auto accident cases, though I am happy to provide craniosacral therapy and/or trigger point therapy to address auto accident injuries. A primary care physician, orthopedist, or other health care provider needs to manage your case, including referrals.

I do not bill out-of-network claims. Payment is required at the time of service. If you have out-of-network benefits that you would like to use, I can provide an itemized receipt that you can submit directly to your insurance to request out of network reimbursement, but any limitations they place on “allowed amounts”are not relevant to out of network providers.
